Yonsei Cancer Center announced on November 7 that it is expanding its heavy-ion therapy program from early-stage non-small cell lung cancer to treat stage 4 oligometastatic lung cancer. Oligometastatic lung cancer refers to a stage 4 diagnosis where tumor spread is limited to a few specific sites rather than widespread across multiple organs, allowing for targeted treatment approaches.
According to clinical research findings, combining systemic treatments like chemotherapy and targeted drugs with local therapies such as radiation to eliminate primary and metastatic tumors can extend patient survival. Recent international phase 3 clinical trials demonstrated that pairing EGFR targeted therapies with radiation to primary and metastatic lesions increased progression-free survival from 12.5 months to 20.2 months, and overall survival from 17.4 months to 25.5 months.
A separate phase 3 clinical trial showed that combining targeted therapy with thoracic radiation therapy extended progression-free survival from 10.6 months to 17.1 months, while overall survival rose from 26.2 months to 34.4 months. Those studies also indicated a 43 percent reduction in disease progression risk and a 38 percent drop in mortality risk.
Domestically, Yonsei Cancer Center evaluated this strategy through a phase 2 multicenter study. The trial paired lazertinib, a third-generation targeted therapy, with stereotactic body radiation therapy for EGFR-mutated oligometastatic lung cancer patients, achieving a median progression-free survival of 34 months without any grade 3 or higher severe radiation pneumonitis.
Targeting High-Risk and Elderly Patients With Precision
Yonsei Cancer Center will use heavy-ion therapy as a local treatment option specifically for older patients or those with compromised lung function who find conventional radiation difficult to tolerate. Patients with underlying conditions like interstitial lung disease or idiopathic pulmonary fibrosis face elevated risks of radiation pneumonitis or worsening pulmonary function under standard radiation protocols.
Heavy-ion therapy delivers high doses of radiation directly to tumor cells while minimizing exposure to surrounding healthy tissue. This precision limits radiation delivery to normal lung tissue as well as critical adjacent organs including the heart and esophagus.
