The 18-month initiative, running through December 2027, will include Wegovy, Zepbound, and Foundayo, but eligibility is restricted to those meeting strict clinical criteria.

To qualify, beneficiaries must have a body mass index (BMI) of 35 or higher, or a BMI of 27 or higher with specific comorbid conditions such as hypertension or sleep apnea. Additionally, participants cannot have conditions treated by GLP-1 drugs already covered under Medicare Part D, like type 2 diabetes, and must not have filled a GLP-1 prescription in 2026. These restrictions narrow the eligible population, as only 3.8 million of the 9.7 million Part D enrollees meeting BMI thresholds ultimately qualify.

At higher participation rates—50% to 75%—the cost could rise to $6.7 billion to $10 billion. These estimates account for a $50 beneficiary copay per prescription, with the government covering the remaining $245 monthly cost.

Obesity affects over 13 million Medicare beneficiaries, but the program’s targeted approach reflects challenges in expanding access to GLP-1 therapies. Unlike broader weight-management initiatives, the Bridge excludes those with diabetes, a group for whom GLP-1s are already covered. This highlights a gap in addressing obesity among Medicare beneficiaries with comorbid conditions, according to the KFF analysis.
