Updated Guidelines Emphasize Early, Aggressive LDL-C Reduction for Cardiovascular Prevention

The Spanish Society of Primary Care Physicians (SEMERGEN), in collaboration with Menarini, has released an updated manual on lipid-lowering treatment, incorporating new scientific evidence to refine dyslipidemia management in primary care settings. This document, updated after a decade, reinforces the critical role of lipid control in cardiovascular prevention, with a continued emphasis on low-density lipoprotein cholesterol (LDL-C) as a primary target due to its direct link to atherosclerosis and cardiovascular events.

Recent clinical trials and meta-analyses have solidified the causal relationship between LDL-C and apolipoprotein B-containing particles in the development of atherosclerotic vascular disease. According to Dr. Antonio Ruiz García, coordinator of the SEMERGEN Cardiovascular Diseases Working Group and an author of the new manual, sustainably reducing these levels can not only lower the risk of heart attack or stroke but may also contribute to reversing the atherosclerotic process.

The updated guidelines maintain a focus on overall vascular risk, integrating new biomarkers and updated evaluation tools. These include the SCORE2 and SCORE2-OP tables, which now utilize non-HDL cholesterol instead of total cholesterol to estimate ten-year cardiovascular risk and extend the applicable age range to individuals aged 70 years and older using SCORE2-OP2.

A key change in the new recommendations is the implementation of LDL-C therapeutic targets based on individual vascular risk levels. Current evidence supports the principle that earlier and more substantial LDL-C reduction leads to improved outcomes, translating into stricter treatment goals.
