Rising Head and Neck Cancer Rates: A Global Concern

A recent global epidemiological study co-led by the Faculty of Medicine of The Chinese University of Hong Kong (CU Medicine) reveals significant disparities in head and neck cancer (HNC) incidence worldwide. While overall incidence in men remains higher, the study highlights a concerning rise in cases among women, particularly in certain regions. Hong Kong’s overall incidence of HNC is also above the global average.

Global Incidence and Trends

The study, conducted in collaboration with universities in mainland China and overseas, found that globally, men are generally diagnosed with HNC at roughly twice the rate of women. However, incidence rates are increasing among women in many parts of the world. Individuals aged 50 to 74 are at a significantly higher risk – approximately eight times greater – compared to those under 50.

In 2022, an estimated 9.8 recent cases of head and neck cancer were reported per 100,000 people globally, with males accounting for approximately three times as many cases as females.1

Regional Disparities

The research identified notable regional variations in HNC incidence. Among the 45 countries analyzed, 14 are experiencing a significant increase in cases, with particularly pronounced rises observed in Spain, Italy, India, the United Kingdom, and China. Conversely, 13 countries are showing a significant decline.

Countries with higher Human Development Index (HDI) scores tend to exhibit lower HNC incidence rates. Researchers suggest this correlation may be linked to increased health awareness and stricter tobacco control measures in these nations. Regions with high smoking rates, such as Oceania and Europe, are experiencing a significant increase in incidence.

Risk Factors and Prevention

Head and neck cancer encompasses malignancies affecting the mouth, throat, nasopharynx, oropharynx, hypopharynx, and salivary glands. Key risk factors include:

  • Smoking
  • Alcohol consumption
  • Chewing betel nut
  • Human papillomavirus (HPV) infection
  • Unhealthy diet, diabetes and dyslipidemia

Public health interventions, such as comprehensive tobacco control policies and strengthened HPV prevention programs, are crucial in reducing HNC incidence.1

Gender-Specific Trends

The study revealed differing trends in HNC incidence based on gender. In 14 countries, the incidence rate among women is increasing, while it is decreasing in 5 countries. For men, the incidence rate is increasing in 12 countries and decreasing in the same number. Some European studies suggest these trends in women may be associated with rising HPV infection rates and an increase in female smokers.

The Demand for Localized Approaches

Researchers emphasize the importance of localized and multidisciplinary approaches to HNC prevention, treatment, and early intervention. Targeted measures are needed to address the increasing risks faced by women and alleviate the global burden of this disease.1
