Elevance Health is actively recruiting a full-time LTSS Service Coordinator – Clinician to serve members in Defiance, Fulton, Henry, Sandusky, or Wood County, Ohio. This field-based role focuses on delivering high-quality, trauma-informed, culturally competent, and person-centered care coordination for members with complex needs.

Role Overview

The LTSS Service Coordinator-Clinician is responsible for a member’s case, contributing to the Long-Term Services and Supports (LTSS) care coordination process. Activities include assisting with assessments to identify, evaluate, coordinate, and manage member needs encompassing physical health, behavioral health, social services, and long-term services and supports, as required by state law and contract. The position requires a current, unrestricted license as an LPN/LVN, LSW, LCSW, or LMSW (or other license allowed by state law).

Key Responsibilities

  • Assist in identifying members at high risk for complications and obtain clinical data as directed.
  • Identify members who may benefit from alternative levels of care or waiver programs.
  • Provide collected information to the responsible Registered Nurse (RN) for verification, interpretation, and development of a member-centered care plan.
  • Participate in coordinating care for members with chronic illnesses, co-morbidities, and/or disabilities, working with the RN, member, and healthcare team to ensure cost-effective and efficient utilization of health benefits.
  • Apply understanding of disease processes and clinical guidelines to decision-making, under the direction of the RN.

Qualifications

Minimum Requirements

  • Requires an LPN/LVN, LSW, LCSW, or LMSW or license other than RN in accordance with applicable state law and Nursing Diploma or AS in Nursing or a related field.
  • Minimum of 2 years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator or similar role.
  • Current, unrestricted LPN/LVN, LSW, LCSW, LMSW or license other than RN (as allowed by state law) in applicable state(s) required.

Preferred Skills & Experience

  • Strong preference for case management experience with older adults or individuals with disabilities.
  • Bachelor’s degree (BA/BS) in Health/Nursing preferred.
