Childhood BMI Linked to Lower Childbirth Rates in Later Reproductive Years

Women who experience higher body mass index (BMI) trajectories during childhood face lower rates of childbirth starting in their twenties, according to a large-scale population-based cohort study published in JAMA Network Open. Researchers found that while childhood weight patterns influence the timing of first childbirth, particularly into later reproductive years, they show little direct correlation with diagnosed clinical infertility.

Childhood BMI Trajectories and Birth Rates

According to the JAMA Network Open study, which evaluated health records from Copenhagen, childhood weight status shapes long-term family planning outcomes. Researchers analyzed data from tens of thousands of women born between 1950 and 1989, tracking reproductive outcomes from 1977 to 2022. The findings reveal that women maintaining an overweight or obese trajectory through childhood exhibit lower childbirth rates by their mid-twenties to late thirties compared to peers with average childhood BMI trajectories. Between the ages of 25 and 29, women with a history of childhood overweight or obesity showed a 9% and 22% lower risk of childbirth, respectively, relative to the reference group. This gap widens substantially past age 30, with women who experienced childhood obesity showing a 44% lower childbirth rate between ages 35 and 45.

Infertility Diagnoses and Healthcare Access

Despite the reduction in live birth rates among women with higher childhood BMI trajectories, the study reported minimal evidence connecting these weight patterns to clinically diagnosed infertility. Among the cohort, approximately 5 381 femmes (9 %) received a primary infertility diagnosis at a median age of 31. Intriguingly, researchers noted that childhood obesity was tied to a 21% lower risk of receiving a primary infertility diagnosis between ages 20 and 45. Study authors explain that this metric likely stems from restricted access to fertility treatments rather than an inherent biological protection against infertility. Under Danish healthcare regulations, fertility services such as in vitro fertilization (IVF) face BMI limits, restricting access for women with a BMI exceeding 30 if they are under 30 years old, or exceeding 35 if they are older.

Public Health Implications and Study Limitations

The findings underscore how early-life metabolic health impacts long-term reproductive timelines well before individuals actively seek fertility evaluations. Because childhood overweight and obesity now affect roughly one in three school-aged girls across Europe and the United States, researchers emphasize the importance of early weight management and healthy growth interventions. However, the study authors acknowledged several limitations, including the lack of adult BMI tracking data to isolate subsequent weight fluctuations and potential disparities in healthcare utilization among women with varying educational backgrounds. These gaps highlight the necessity for continued research into the physiological and social drivers that influence reproductive milestones in modern populations.
