Low- and no-calorie sweeteners can aid weight management following rapid weight loss without increasing the risk of diabetes or cardiovascular disease, according to new research presented at the European Congress on Obesity held from May 14 to 16, 2024, in Venice, Italy. The findings emerge from the extensive SWEET project, a one-year trial designed to evaluate whether substituting sugar with sweeteners helps adults and children maintain weight loss while improving diet satisfaction and reducing sweet cravings.
The SWEET Project Trial Design and Participant Demographics
The research involved 341 adults with overweight or obesity alongside 38 children categorized as overweight, hailing from Denmark, Spain, Greece, and the Netherlands, according to trial details discussed by Medical News Today. During the initial two-month phase of the study, adult participants adhered to a strict low-calorie diet with the primary objective of losing at least 5% of their baseline body weight, while participating children focused on maintaining their current weight. Following this initial rapid weight loss phase, researchers divided the participants into two distinct groups for the subsequent 10 months.
Group A followed a healthy diet deriving less than 10% of its total calories from added sugars while incorporating foods and beverages containing low- or no-calorie sweeteners and sweetness enhancers. Group B followed the exact same healthy low-sugar diet regimen entirely without the use of sweeteners. Throughout the duration of the 12-month study, participants completed regular questionnaires detailing their dietary intake, eating habits, physical activity levels, and overall quality of life. Researchers also tracked participant body measurements, weight fluctuations, and established clinical biomarkers for diabetes and heart disease at baseline and at 2, 6, and 12 months.
Psychological and Dietary Effects of Sweetener Consumption
Participants who utilized sweeteners reported notable psychological and behavioral benefits during the trial. Clarissa Dakin, co-lead author of the SWEET project, stated that after six months of the intervention, the group incorporating sweeteners and sugar substitutes experienced greater diet satisfaction, more positive moods, and fewer cravings for sweet foods compared to the abstaining group.
However, long-term tracking revealed a shift in behavioral patterns. Dakin noted that after 12 months, participants in the group that strictly avoided sweeteners and sugar substitutes showed an increased liking for other calorie-containing sweet foods. Furthermore, the data showed that individuals who included sweeteners during the trial maintained their weight slightly better at the one-year mark than those in the non-sweetener group. Participant perceptions also evolved over the course of the study. Dakin explained that those who included sweeteners tended to view them more positively regarding health outcomes by the conclusion of the trial, whereas participants who avoided sweeteners became less positive about artificial options and somewhat more positive about the safety of regular sugar.
Cardiovascular and Metabolic Safety Outcomes
A primary objective of the SWEET project was to evaluate the safety profile of sugar substitutes on metabolic health parameters. According to the findings presented in Venice, researchers found no significant differences in clinical markers for diabetes and cardiovascular disease between the group consuming sweeteners and the group avoiding them. Natural and artificial sweeteners—including aspartame, stevia, and saccharin—did not elevate disease risks over the one-year monitoring period.

Despite these promising results, experts emphasize that questions remain regarding younger populations. Because the pediatric cohort included only 38 children, researchers stressed that additional large-scale studies involving children are necessary to conclusively determine the long-term physiological and behavioral effects of sweeteners on younger individuals. While the findings support the utility of sugar substitutes in adult weight management protocols, ongoing scientific investigation continues to explore how these additives interact with varied dietary patterns across different age demographics.
