A newly developed microRNA-based liquid biopsy accurately detects early-stage pancreatic cancer by analyzing small bits of RNA released by tumors into the bloodstream, according to findings presented at the American Association for Cancer Research annual meeting. Developed by Ajay Goel, Ph.D., of the City of Hope Duarte Cancer Center in California, alongside colleagues, the blood test addresses a major diagnostic hurdle in oncology: catching one of the world’s deadliest cancers before it turns invasive and spreads to other organs.

How the MicroRNA Liquid Biopsy Works

For the vast majority of patients, pancreatic cancer is diagnosed only after it has crept into surrounding organs, rendering surgical removal impossible. Early-stage disease rarely causes obvious symptoms, and traditional diagnostic tools lack reliability. According to the National Cancer Institute (NCI), the 5-year survival rate sits at 44% for early-stage disease compared to just 3% for late-stage diagnoses. The new liquid biopsy aims to shift those statistics by capturing tumor-derived RNA signals from a standard blood draw.

For over a decade, scientists have studied microRNAs as potential cancer biomarkers because tumors release them in massive quantities, and they remain stable in circulation. Free-floating microRNAs are easy to measure, but they typically lack a tissue-of-origin indicator. According to researchers, these exosomes carry distinct organ-specific markings that act like a ZIP code, allowing the assay to confirm whether the circulating genetic material originated in the pancreas.

Clinical Accuracy and High-Risk Screening Populations

The international study evaluated nearly 1,000 individuals across multiple countries, demonstrating that the assay successfully identifies both early- and late-stage pancreatic malignancies in a diverse patient cohort. Furthermore, when researchers paired the microRNA assay with an existing test for the protein marker CA19-9, the combined approach correctly identified 97% of patients with early-stage disease.

Despite the high accuracy rates, experts emphasize that broad population-wide screening remains impractical for now. Matthew Young, Ph.D., co-lead program director of the NCI’s Pancreatic Cancer Detection Consortium—which funded the study—explained that deploying the test in the general population would generate a high volume of false positives. Because a positive screening result typically triggers invasive abdominal surgery, researchers recommend a targeted approach. Howard Crawford, Ph.D., scientific director of the Henry Ford Pancreatic Cancer Center in Michigan, noted that the technology requires additional validation before entering standard clinical practice. Both Dr. Goel and Dr. Young agree that the most effective initial application of the test involves high-risk individuals, such as patients with inherited genetic mutations, a strong family history of the disease, chronic pancreatitis, or new-onset diabetes.
