Private Health Insurance Premiums Set to Rise for Retirees in January 2027

Private health insurance premiums for retirees in Germany will increase significantly starting January 1, 2027, according to data released by the PKV-Verband and analyzed by rentenbescheid24.de. Approximately 53 percent of private health insurance policyholders will face higher monthly costs at the start of the year. The scope of the increase varies depending on individual beneficiary status and specific tariff structures. Individuals receiving government employee assistance, known as Beihilfeberechtigte, face an expected average increase of 11.4 percent. For all other affected policyholders, premiums will climb by an average of 15.3 percent. Because rates depend heavily on individual contracts, actual financial impacts will differ widely across the insured population.

Calculating the Impact on Monthly Retirement Budgets

The percentage increases translate directly into substantial cash outlays for retirees living on fixed incomes. For a policyholder currently paying a monthly premium of 500 euros, a 13.4 percent increase adds roughly 67 euros each month. A 700-euro monthly premium rises by 93.80 euros under the same average adjustment. Policyholders paying 900 euros per month will see their baseline costs increase by 120.60 euros monthly.

According to the PKV-Verband, the upward pressure on premiums stems primarily from rising expenditures for hospital treatments, prescription drugs, therapeutic remedies, and outpatient medical services. However, insurers cannot raise rates arbitrarily. Premium increases are legally and contractually restricted, and insurers may only implement them if specific statutory requirements within each distinct tariff are fully satisfied.

Retirees Can Apply for Statutory Pension Insurance Subsidies

Retirees covered by private health insurance can offset some of these higher costs by applying for a designated subsidy from the German statutory pension insurance scheme, Deutsche Rentenversicherung, pursuant to Section 106 of the Sixth Book of the Social Code (§ 106 SGB VI). The exact amount of this financial support depends on factors such as the statutory pension amount, the general contribution rate, and the average additional contribution. The subsidy is legally capped at half of the actual private health insurance expenses incurred by the policyholder. Weil der Zuschuss bisher wegen des niedrigeren PKV-Beitrags begrenzt war, kann die Erhöhung zum 1. Januar 2027 Spielraum für einen höheren Zuschuss schaffen. Affected individuals can verify their eligibility and formally request the adjusted subsidy from the Deutsche Rentenversicherung using official Form R0820.

Evaluating Internal Tariff Switches Under Section 204 VVG

Beyond securing a higher statutory pension subsidy, policyholders can review their options for an internal tariff switch according to Section 204 of the Insurance Contract Act (§ 204 VVG). Opting for a cheaper tariff can ultimately prove costly if the switch results in the elimination of essential medical coverages.
