The CFTR gene produces a protein that regulates the movement of salt and water in and out of cells. When this protein is defective, it leads to the production of abnormally thick and sticky mucus that can clog airways and interfere with nutrient absorption.

Symptoms of Cystic Fibrosis

Symptoms of CF primarily affect the lungs and digestive system and can vary in severity. Common symptoms include:

  • Persistent cough with thick mucus

Diagnosing Cystic Fibrosis

This screening usually involves measuring levels of immunoreactive trypsinogen in a blood sample, followed by genetic testing to identify CFTR mutations. The gold standard for diagnosis is a sweat chloride test, which measures the amount of chloride in sweat. A value of 60 or higher indicates CF.

It’s important to note that CF can sometimes be diagnosed in adulthood, particularly in individuals who were not screened as newborns or who have milder forms of the disease.

Treatment Options for Cystic Fibrosis

Treatment for CF is complex and requires a multidisciplinary team of healthcare professionals, including pulmonologists, gastroenterologists, dietitians, and respiratory therapists. Treatment focuses on managing symptoms, preventing complications, and improving quality of life.

Airway Clearance

For individuals not eligible for specific CFTR modulator therapies, airway clearance techniques are crucial. These techniques assist loosen and remove mucus from the lungs, reducing the risk of infection. Common methods include manual chest physical therapy, huff coughing, high-frequency chest vests, and breathing exercises.

Medications

Various medications are used to manage CF, including:

  • Antibiotics: To treat and prevent lung infections.
  • Bronchodilators: To open airways.
  • Pancreatic enzymes: To aid in digestion and nutrient absorption.
  • CFTR Modulators: These medications, such as elexacaftor-tezacaftor-ivacaftor (Trikafta), target the underlying cause of CF by helping the defective CFTR protein function more effectively. Trikafta is currently approved for individuals with at least one F508del mutation or other responsive CFTR gene mutations and is available for those 2 years or older.

Nutritional Support

Maintaining a healthy weight and receiving adequate nutrition are essential for individuals with CF. A high-calorie, balanced diet, along with pancreatic enzyme supplementation, can help ensure proper nutrient absorption.

Treatment is best managed at a Cystic Fibrosis Foundation (CFF)-accredited care center. These centers follow a standard model of care and offer comprehensive, specialized treatment. They also conduct clinical research to advance scientific knowledge and improve outcomes for individuals with CF.
