Dr. Tommy Wood, a professor of pediatrics and neuroscience at the University of Washington, cautioned that extreme dietary deficits during treatment risk harming the brain’s physical structure and cognitive resilience.

Brain Energy Availability and the Risks of Rapid Deficits

The brain functions as the body’s most energy-hungry organ, relying on a continuous supply of fuel to maintain its tissue integrity. According to Dr. Wood, who spoke at New York City’s Strong NY fitness and wellness festival, brain reserve—the physical amount and integrity of brain tissue—follows a bell-shaped curve relative to the body’s energy availability.

Excess energy intake associated with obesity, high blood sugar, and metabolic disease damages blood vessels and drives insulin resistance, which lowers overall brain resilience. In these cases, GLP-1 medications offer cognitive benefits by reducing blood pressure and improving blood sugar control.

However, the lower end of the energy spectrum introduces structural risks. Severe caloric restriction can force the brain to consume its own tissue when fuel supplies plummet. Dr. Wood pointed to studies examining extreme endurance events, such as a marathon, where intense energy demands caused temporary shrinkage in the brain’s white matter. While those participants recovered after resting and refueling, the findings illustrate that the brain will decrease in size when starved of adequate energy.

Muscle Quality Versus Lean Mass Loss During Treatment

Concerns regarding lean muscle loss during GLP-1 therapy require careful clinical distinction. Dr. Wood noted that some of the initial "lean mass" lost during treatment is actually fat that had accumulated inside the muscle tissue.

While muscles may become physically smaller, patients do not necessarily grow weaker. In many instances, muscle quality and functioning actually improve as metabolic health is restored.

Protecting Central Nervous System Health With Neuro-Nutrients

Risks emerge when individuals maintain extreme caloric deficits over extended periods. Chronic under-eating deprives the central nervous system of structural building blocks necessary for long-term cognitive function.

Dr. Wood emphasized that patients must consume adequate core neuro-nutrients to prevent cognitive impairment. Essential dietary components include:

  • B vitamins, specifically B12, folate, and B6
  • Vitamin D
  • Iron
  • Omega-3 fatty acids
  • Protein

Without sufficient intake of these elements, patients risk compromising neural repair and maintenance. To safeguard both brain and muscle health during a weight-loss regimen, Dr. Wood recommends pairing GLP-1 therapy with resistance training twice a week. This approach stimulates muscle retention and promotes growth factors within the central nervous system, ensuring patients lose weight without pushing their physiological systems into a damaging deficit.
