Phase 2 ALTITUDE-AD Trial Misses Primary Endpoint

Acumen Pharmaceuticals reported on October 30, 2026, that its Phase 2 ALTITUDE-AD clinical trial evaluating sabirnetug in early Alzheimer’s disease missed its primary endpoint. Acumen Pharmaceuticals, Inc. announced that neither the 35 mg/kg nor the 50 mg/kg dose improved cognitive and functional decline compared to placebo on the Integrated Alzheimer’s Disease Rating Scale at 18 months.

Secondary Endpoints and Safety Findings

Key secondary efficacy endpoints in the study also failed to achieve statistical significance. The trial showed no meaningful differences between either treatment arm and the placebo arm on measures such as the Clinical Dementia Rating – Sum of Boxes, according to the company statement. On safety, the company stated that sabirnetug was generally well tolerated. Amyloid-related imaging abnormalities involving brain swelling occurred in approximately 10% of participants treated with the drug. Meanwhile, an analysis of positron emission tomography scans demonstrated statistically significant reductions in amyloid plaque load for both dose levels at week 36 and week 76.

Strategic Evaluation and Cash Conservation Plans

Following the trial results, Acumen plans to cease clinical development of sabirnetug, which includes ending the open-label extension portion of the ALTITUDE-AD study. The company stated that it will implement cash conservation measures and evaluate strategic options, including its Enhanced Brain Delivery program. “While we are disappointed the trial did not meet its primary endpoint, we express our compassion and hope for future therapeutic options for patients with Alzheimer’s disease and their families,” said Daniel O’Connell, Chief Executive Officer of Acumen, in the release.
