Novel Drug-Pad IOL System Improves Glaucoma and Cataract Management
Published: March 13, 2026
A recent drug-eluting intraocular lens (IOL) system, the Bimatoprost Drug Pad-IOL System (BIM-IOL System), has demonstrated promising results in improving intraocular pressure (IOP) and vision while reducing treatment burden for patients with open-angle glaucoma or ocular hypertension undergoing cataract surgery. Phase 1/2 trial data revealed significant IOP reductions and vision improvements with two different dose levels of the system.
Study Details and Findings
The phase 1/2 trial involved 104 patients who were randomly assigned to one of three groups: 78 µg BIM-IOL System with artificial tear drops (51 patients), 39 µg BIM-IOL System with artificial tear drops (23 patients), or a control group receiving a commercially available monofocal IOL with timolol eye drops (30 patients).
At 12 months, patients in the 78 µg and 39 µg dose groups experienced 34% and 42% reductions in mean IOP, respectively, measured at both 8 a.m. And 10 a.m. The control group showed a 35% reduction in mean IOP at 8 a.m.
Notably, a significant proportion of patients treated with the BIM-IOL system were able to discontinue all topical IOP-lowering medications. 98% of patients in the 78 µg group and 96% in the 39 µg group were free from these medications.
All evaluable patients experienced vision improvements, achieving a best-corrected distance visual acuity (BCDVA) of 20/32 or better, with a mean BCDVA of 86 letters.
Safety Profile
The BIM-IOL system demonstrated a safety profile comparable to routine cataract surgery, with no serious ocular adverse events reported.
Future Directions
“This readout clearly demonstrates the potential for SpyGlass technology to have a life-changing impact on patients while being accessible to 100% of cataract surgeons,” said Patrick Mooney, CEO of SpyGlass Pharma. “We’re now in a category of our own in demonstrating our ability to eliminate topical IOL-lowering medications for the vast majority of patients while delivering the same high quality of vision that patients and surgeons expect from state-of-the-art IOLs.”
SpyGlass Pharma is currently focused on enrolling patients in two pivotal phase 3 trials of the BIM-IOL System.
