The emergence of a new variant of the mpox virus has brought the issue back into focus for international health surveillance. The recent identification of two cases linked to a recombinant strain underscores the importance of ongoing monitoring of viruses that continue to circulate globally, even outside of active outbreak periods.

What is a Recombinant Mpox Strain?

The newly identified variant results from a recombination between two known clades of the mpox virus: clade Ib and clade IIb. Recombination is a natural biological process where two genetically similar viruses infect the same individual simultaneously and exchange genetic material.

This process can create variants with novel characteristics, though it doesn’t automatically imply increased severity or transmissibility. Currently, the limited number of cases prevents definitive conclusions about whether the new strain causes different clinical outcomes or spreads more easily.

Confirmed Cases and What They Indicate

Both confirmed cases involved individuals with recent international travel histories. The first case was detected in the United Kingdom in December 2025, in a traveler returning from a country in Southeast Asia. The second case was identified in India, with the individual having traveled through the Arabian Peninsula.

Detailed genomic analysis revealed that both patients were infected with the same recombinant strain, despite being diagnosed weeks apart. This suggests the variant may not be an isolated incident but part of a broader, currently undetected circulation. Importantly, contact tracing in both countries did not identify any secondary transmissions, indicating no sustained spread to date.

Risk Assessment and Most Exposed Groups

The World Health Organization (WHO) maintains that the overall global public health risk remains unchanged. The risk is currently assessed as low for the general population without specific risk factors. However, it remains moderate for groups with higher exposure, particularly men who have sex with men with new or multiple partners, and for sex workers or individuals with frequent close physical contact.

This pattern suggests that mpox transmission dynamics remain similar to those observed in recent years, with no clear evidence of changes linked to the new variant.

Vaccination and Ongoing Monitoring

Current immunization strategies against mpox, based on vaccines originally developed for smallpox, continue to demonstrate good effectiveness. These vaccines are still recommended, especially for priority groups and individuals at higher risk of exposure.

With the identification of this new variant, health authorities emphasize the following preventive actions:

  • Strengthening epidemiological surveillance
  • Expanding genetic sequencing of the virus
  • Targeted immunization of vulnerable populations
  • Strict implementation of infection prevention and control measures

The emergence of a recombinant strain doesn’t necessarily signal a new large-scale outbreak, but it highlights the ongoing genetic evolution of the virus. Early case detection and systematic monitoring remain the most effective tools for mitigating risks and protecting public health.
