Drugs like Ozempic and Wegovy, initially developed for weight loss and diabetes management, are showing promise in improving recovery rates after a heart attack. New research suggests these GLP-1 medications may help prevent further heart damage by restoring blood flow to the heart muscle.

Understanding the Problem: ‘No-Reflow’ After a Heart Attack

When someone experiences a heart attack, a blockage occurs in a coronary artery, restricting blood flow to the heart. While doctors typically work to quickly open the blocked artery, restoring full blood flow isn’t always immediate. This can lead to a condition known as “no-reflow,” where tiny blood vessels called capillaries remain constricted, hindering oxygen delivery to heart tissue and increasing the risk of complications, heart failure, or even death 1.

How GLP-1 Drugs May Help

Researchers at the University of Bristol and University College London (UCL) have discovered that GLP-1 drugs may help relax the cells surrounding these capillaries, allowing blood to flow more freely. Their experiments, conducted on surgically removed mouse hearts, focused on cells called pericytes, which wrap around capillaries and can constrict them during a heart attack 2.

The study found that GLP-1 activates ion channels in the pericyte cell membrane, causing them to relax and restore the capillaries to their pre-heart attack state 1. This discovery may explain how a related procedure, remote ischemic conditioning (RPc), which triggers a protective response in the brain to open capillaries, works – as RPc is known to release GLP-1 2.

GLP-1 Drugs and Cardiovascular Risk

The potential benefits of GLP-1 drugs extend beyond heart attack recovery. Recent research indicates that staying on GLP-1 medications can reduce cardiovascular risk in patients with Type 2 diabetes. A study following over 333,000 adults with diabetes found that those who consistently took GLP-1s experienced an 18% reduction in cardiovascular risk over three years 3.

Conversely, even short interruptions in treatment—as little as six months—can diminish these protective effects. A two-year gap in treatment increased cardiovascular risk by 22% compared to continued use 3.

What are GLP-1 Drugs?

GLP-1 drugs mimic the natural glucagon-like peptide-1 (GLP-1) hormone, which plays a role in regulating digestion, appetite, and blood sugar levels. Commonly prescribed GLP-1 medications include semaglutide (Ozempic, Wegovy) and liraglutide (Victoza, Saxenda) 4.

Future Research and Implications

While these findings are promising, it’s significant to note that the initial research was conducted on mouse hearts. Further studies are needed to determine if Ozempic, Wegovy, and other GLP-1 drugs have the same capillary-relaxing effect in humans 1. Yet, researchers suggest that these readily available drugs could potentially be repurposed to treat ‘no-reflow’ in heart attack patients, offering a life-saving solution 2.
