Spain Authorizes Historic Euthanasia for Depression and Physical Pain

Barcelona, Spain – In a landmark and deeply divisive case, Noelia Castillo Ramos, a 25-year-old woman paralyzed after a traumatic assault, underwent euthanasia today, Thursday, March 26, 2026, following a protracted legal battle. This marks the first instance of euthanasia being authorized in Spain for a patient suffering from both severe depression and physical pain.

A Life Altered by Trauma

Noelia Castillo Ramos’s life changed dramatically in 2022 after surviving a fall from a fifth-floor window following a reported multiple sexual assault. The fall resulted in paraplegia, leaving her with chronic and debilitating pain and profound emotional suffering. Prior to the fall, she had also experienced a previous sexual assault by an ex-boyfriend and overdosed on medication, and had been receiving psychiatric treatment since the age of 13, being diagnosed with obsessive-compulsive disorder (OCD) and borderline personality disorder (BPD). She repeatedly expressed a desire to conclude her suffering, stating she wished to “rest.”

Legal Battles and Family Opposition

While Noelia’s request for euthanasia was initially approved under Spain’s legislation, which permits assisted dying for individuals with serious and incurable conditions or intense suffering, the process was far from straightforward. Her father challenged the decision in court, arguing that her mental health impaired her capacity to craft an informed decision about ending her life. The legal battle spanned nearly two years, involving multiple appeals and ultimately reaching the European Court of Human Rights, which rejected the father’s request for interim measures to halt the procedure on March 10, 2026. A final attempt by the father’s legal team on Wednesday to introduce “urgent precautionary measures” was also denied by a Barcelona court due to lack of jurisdiction.

Noelia’s Final Wishes

In her only interview, recorded at her maternal grandmother’s house and broadcast on Spanish TV program And Now Sonsoles, Noelia expressed her desire for a peaceful death. She stated she wanted to “die looking pretty,” and planned to wear her “prettiest dress” and apply makeup. She requested to be alone in her bedroom at the care facility during the procedure, and wished to have four photos with her: one of her painting a portrait of her mother, one of her childhood puppy, another from her first day of school, and a fourth childhood photograph representing happy memories.

Spain’s Euthanasia Law

Spain legalized euthanasia and assisted suicide in 2021, becoming the fourth European Union country to do so. The law allows individuals with incurable or severely debilitating conditions to request assistance in ending their lives. The legislation has been met with significant opposition from conservative groups and the Catholic Church, sparking ongoing debate about end-of-life rights and personal autonomy.
