2024 ESC Congress Highlights: Breakthroughs in Heart Care and Equity
The 2024 European Society of Cardiology (ESC) Congress was a hub of groundbreaking research, showcasing innovative approaches to cardiovascular care across screening, treatment, and health equity. Traditional Screening May Miss High-Risk Patients
A study presented by Anna Mueller, MD, and Amir Ahmadi, MD, of Mount Sinai, revealed a concerning gap in traditional screening methods for acute coronary syndrome (ACS). Retrospective data from 166 patients under 65 revealed that 51% of those who experienced their first ACS event were classified as low or borderline risk using the Atherosclerotic Cardiovascular Disease (ASCVD) Risk Score. Furthermore, over half of the patients had no symptoms until less than 48 hours before their event, with some experiencing no symptoms whatsoever.
“These findings highlight the limitations of relying solely on symptoms and risk scores for ACS identification,” said Mueller and Ahmadi in an exclusive interview. “Advanced imaging, such as Coronary CT Angiography, can detect coronary plaque, enabling earlier risk identification and potentially preventing heart attacks.”

Lipid-Lowering Therapies Advance Beyond Statins
Three key studies explored advanced lipid-lowering therapies for patients who aren’t achieving optimal low-density lipoprotein (LDL) cholesterol levels with statins alone.
- Bempedoic acid showed significant reductions in major adverse cardiovascular events, primarily by lowering nonfatal myocardial infarctions. However, it did not affect all-cause mortality.
- A meta-analysis revealed that combining statins with ezetimibe, PCSK9 inhibitors, or bempedoic acid reduced cardiovascular events and ischemic strokes. While beneficial, these combinations didn’t improve overall mortality.
- A study highlighted the gap between lipid-lowering guidelines and real-world implementation in the United Kingdom, showing that most high-risk patients failed to achieve LDL targets or receive appropriate combination therapies.
Screening for Coronary Plaque: A Paradigm Shift in Heart Disease Detection
Research presented at the ESC Congress suggests a paradigm shift in heart disease screening, moving away from ASCVD scores and towards detecting coronary plaque.
“Nearly 40% of patients presenting with ACS were classified as low-risk and would have been missed by symptom-based assessments alone,” emphasized Dr. Mueller and Dr. Ahmadi in their discussion.
They advocated for advanced imaging techniques, such as CT scans, to identify subclinical coronary disease earlier, comparing the approach to cancer screening. Early detection through plaque identification can potentially prevent future heart attacks.

Advancing Health Equity in Cardiovascular Care
