The Evolving Landscape of Sexually Transmitted Infections: Prevention, Diagnosis, and Treatment

Sexually transmitted infections (STIs) are experiencing a concerning resurgence, despite once being considered largely under control in Western countries. This rise is driven by the remarkable adaptability of the microbes responsible for these infections, which constantly mutate and evolve, sometimes becoming more difficult to treat. Alongside these biological shifts, advancements in screening, prevention strategies, and a growing emphasis on accessible care are reshaping the response to STIs.

The Challenge of Microbial Adaptation

Bacteria, viruses, and fungi possess a remarkable capacity for adaptation. They mutate, alter their transmission methods, and, crucially, can develop resistance to treatments. Dermatologist and venereologist Sébastien Fouéré highlights this ongoing dynamic, noting that innovation in STI management often involves adapting to microbial evolution rather than solely relying on new drugs.1

Recent trends underscore this challenge. The emergence of mpox (formerly monkeypox) demonstrated how infections can modify transmission routes, spreading through prolonged intimate contact. Research is ongoing to characterize fungal infections potentially transmitted sexually, revealing the fluidity of the STI landscape.1 Perhaps most concerning is the increasing prevalence of bacterial resistance, particularly in infections caused by gonococci and Mycoplasma genitalium, where bacteria are developing defenses against antibiotics, diminishing treatment effectiveness.1

Simplifying Screening for Early Detection

Early diagnosis remains a cornerstone of STI control, limiting both transmission and potential complications like infertility or reproductive health issues. Historically, barriers to screening – including cost, prescription requirements, and fear of judgment – hindered access. Recent initiatives aim to address these obstacles.

In France, the “Mon test IS” program, launched in September 2024, provides free, prescription-free STI screening for individuals aged 18 to 26 at medical biology laboratories.1 This standardization of screening aims to promote early diagnosis. Although diagnostic tools themselves – PCR tests for detecting bacterial or viral DNA and serologies for identifying infections in the blood – haven’t drastically changed, increased accessibility is a significant breakthrough.1

Centres de Dépistage et d’Information des Infections Sexuellement Transmissibles (CeGIDDs) throughout France also play a vital role, offering free, often anonymous, screening, treatment, and support.1

Prevention Strategies: A New Era

While treatment remains crucial, the focus is shifting towards prevention. Several advancements are transforming this area.

Vaccination

Vaccination is a powerful preventative tool. Vaccines are available for hepatitis B (sexually transmitted), hepatitis A (in certain exposure situations), and human papillomavirus (HPV). The HPV vaccine protects against genital warts and several cancers, including cervical, anal, and oropharyngeal cancers.3

PrEP (Pre-Exposure Prophylaxis)

HIV prevention has been revolutionized by pre-exposure prophylaxis (PrEP), an antiretroviral treatment taken before potential exposure to the virus. PrEP effectively prevents viral establishment in the body when used correctly.1 Two methods exist: a daily dose for those regularly at risk and an “on-demand” dose timed around risky sexual encounters.1

Doxycycline Post-Exposure Prophylaxis (Doxy-PEP)

Research is exploring doxycycline post-exposure prophylaxis (doxy-PEP) – taking the antibiotic doxycycline after unprotected sex – to reduce the risk of bacterial infections like syphilis and chlamydia.2, 4 Studies have shown that 200 mg of doxycycline taken within 72 hours of sex can reduce syphilis and chlamydia infections by over 70% and gonococcal infections by approximately 50%.4 However, the French High Authority of Health has not yet recommended widespread use, pending further scientific data.1

A Holistic Approach to STI Control

No single strategy is sufficient. A comprehensive approach combining condoms, regular screening, vaccination, information, and accessible care is essential. This approach must be tailored to individual circumstances. Addressing STIs requires open conversations about sexuality, reducing stigma, and ensuring prevention is accessible to all.
