Nasal Swab Test Detects Early Alzheimer’s Signals, Study Finds
Diagnosing Alzheimer’s disease in its earliest stages, when treatment can be most effective, remains a major challenge. Scientists at Duke Health have unveiled a revolutionary approach: a quick and painless nasal swab that can detect biological signs of the disease before memory decline. Duke Health researchers demonstrated that the test can pick up early biological changes linked to Alzheimer’s, even before thinking and memory problems appear.
How the Nasal Swab Test Works
The researchers leverage a soft brush to collect cells from the upper part of the nasal cavity, where the olfactory neurons are located. Analysis of gene activity in these cells revealed clear molecular differences between healthy people and those with early or clinical signs of Alzheimer’s. The procedure takes just a few minutes and involves applying a numbing spray before a clinician gently guides the brush into the nose.
Key Findings of the Study
Published March 18, 2026, in Nature Communications, the study compared samples from 22 participants, measuring the activity of thousands of genes across hundreds of thousands of individual cells. The analysis focused on nerve and immune cells, identifying patterns that distinguished individuals with early or diagnosed Alzheimer’s from those without the disease.
- Immune Cell Activation: CD8 T cells, which drive inflammation, showed 13.4% activation in preclinical Alzheimer’s subjects compared to just 4.1% in healthy controls.
- Neuronal Gene Shifts: The activity of certain genes linked to brain inflammation (AKR1B1) increased, while genes involved in cellular stress response (CERT1) decreased.
The Importance of Early Detection
As lead author Bradley Goldstein, M.D., Ph.D., noted, the goal is to confirm the disease before irreversible changes occur in the brain. Early diagnosis paves the way for treatments that can potentially prevent the development of severe symptoms. The method showed high accuracy, correctly identifying the disease in approximately 81% of cases, particularly in individuals with laboratory markers of Alzheimer’s but no visible clinical manifestations.
A New Perspective on Alzheimer’s Research
Unlike blood tests, which reveal later-stage markers, a nasal swab provides a unique opportunity to study living nerve and immune cells that reflect the earliest pathological processes. As study co-author Vincent D’Anniballe emphasized, much of our current knowledge about Alzheimer’s has been obtained posthumously. This new method allows researchers to study living nervous tissue in patients for the first time, opening new horizons for diagnosis and drug development.

Volunteer Mary Umstead was inspired to participate in the study by a personal tragedy – her sister’s illness. She hopes the scientists’ work will help other families avoid similar pain.
Future Directions
The research team is now expanding the study to larger groups of people and testing whether the test can track the effectiveness of therapies. A patent application has already been filed for the development.
