Omvoh Receives FDA Approval and Shows Promise for Crohn’s Disease Patients

A new treatment option, Omvoh (mirikizumab-mrkz), is now available for adults with moderate to severe Crohn’s disease who have not responded adequately to conventional therapies. Recent data demonstrates its efficacy in maintaining steroid-free remission and reducing the need for surgery and hospitalization.

Understanding Crohn’s Disease

Crohn’s disease is a chronic inflammatory bowel disease (IBD) that causes inflammation of the digestive tract. It can affect any part of the gastrointestinal tract, from the mouth to the anus, but commonly affects the ileum (the end of the small intestine) and colon. Inflammation often occurs in discontinuous patches and can penetrate the entire thickness of the intestinal wall, leading to complications like fistulas and stenosis.

In Spain, the overall adult incidence of IBD is approximately 16.2 cases per 100,000 inhabitants per year, with Crohn’s disease accounting for 7.4 cases per 100,000 inhabitants per year .

How Omvoh Works

Omvoh reduces inflammation in the gastrointestinal tract by targeting interleukin-23 p19, a protein crucial in the development of intestinal inflammation. It is the first biologic in over 15 years with two-year phase 3 efficacy data at the time of approval .

Clinical Trial Results: Durable Remission

Clinical trials have shown significant benefits with Omvoh:

  • Long-Term Remission: Over 90% of Crohn’s disease patients who achieved steroid-free remission after one year maintained it through three years of treatment .
  • Reduced Complications: Patients treated with Omvoh experienced lower rates of surgery and hospitalization. There was a nearly 50% reduction in disease-related complications compared to placebo within the first 12 weeks .
  • Initial Efficacy: The VIVID-1 study demonstrated that 53% of patients achieved clinical remission at one year with mirikizumab, compared to 36% on placebo .
  • Endoscopic Response: The VIVID-2 study indicated that over 80% of patients maintained an endoscopic response after one year with continued treatment .
  • Durable Efficacy: New long-term data from the VIVID-2 open-label extension study showed durable efficacy through three years .

Managing Crohn’s Disease: Lifestyle and Prevention

Although Crohn’s disease cannot be entirely prevented, certain lifestyle factors can help reduce risk or severity:

  • Quit Smoking: Smoking significantly worsens Crohn’s disease, increases relapses, and raises the likelihood of surgery.
  • Healthy Diet: A Mediterranean-style diet rich in fiber (fruits, legumes, vegetables) and low in ultra-processed foods is beneficial.
  • Maintain a Healthy Weight
  • Judicious Antibiotic Use: Avoid unnecessary antibiotic use.
  • Limit NSAIDs: Use nonsteroidal anti-inflammatory drugs (NSAIDs) like ibuprofen and naproxen only when essential.

The Future of Crohn’s Disease Treatment

The current focus in Crohn’s disease treatment is shifting towards early intervention and achieving clear objectives, such as mucosal healing and low biomarkers of inflammation. This “treat-to-target” strategy aims to control inflammation long-term, reduce complications, and improve patients’ quality of life. Future advancements are expected to include faster diagnoses, more personalized treatments, and close monitoring within multidisciplinary teams.
